USC Team swim caps can be ordered by completing the CAP ORDER FORM and mailing it along with a check payable to University Swim Blub to Nikki Hetrick 420 Chardon Ave, Chardon OH 44024 

If you wish to order a personalized USC cap with the swimmer's name, your order must be received by 10/28/11. 

USC caps with no personalization can be purchased after the deadline on a first come first served basis until they run out.
